PURPOSE: This form allows the PTFC Board of Directors to follow a regimented process for the review and selection of volunteers to fill coaching positions for the upcoming PTFC season. Prior to any individual being selected for a position as a Head or Assistant Coach they must submit a completed application, agree to a background and CORI check and agree to the possibility of an interview. The information collected will only be used as the basis for review and ultimate recommendation to the PTFC Board of Directors for final approval of appointment of the applicant to participate in a coaching position. We do not anticipate challenges; however, we are committed to maintaining a quality and safe environment for all participants. All results will remain confidential. 

 

INSTRUCTIONS: This application must be returned no later than April 30th, 2025. Applications received after this date will not be guaranteed consideration for a coaching position for the upcoming season. The PTFC Board of Directors will consider and appoint candidates until all positions are filled. If any of the items below need further explanation or entry space, please email PTFC02359@gmail.com. Your application will not be considered if it is not complete and accurate.
